Name & Address ChangeRequest for Name & Address ChangesChanges to the personal information of an academic record should be requested in writing (on paper or electronically) and directed to the Office of Admissions & Records. Verification of a legal name change must be provided before academic records can be updated. Documents accepted as proof of a name change include birth certificates, marriage licenses, divorce decrees authorizing the name change, judicial decrees specifically authorizing the name change, and naturalization papers. Current and former employees of the University should submit their requests for Change of Personal Information to Human Resources at the Wheeling campus. Online Change of Address Change your address online! Current students may now make address changes by logging into your portal account, selecting the "My Services" tab in the portal, selecting the sentence link mid-page, and then choosing "Personal Information". You may not utilize this feature if you have ever been an employee of the university. Go to MY.NL.EDU to login. |
