Registration Procedure
for Chicago Public School teachers

Application & Registration

• Two forms are needed:

  • Application form from Chicago Public Schools

  • Registration form from National-Louis University — including a credit card section that may be used for the student payment of $455.

  • The forms (in PDF format) are available by following these links:

• Payment Options

  • Credit card registration and application may be faxed to 847-465-5629,
    ATTN: Center for Professional Development

  • Checks / money orders may be mailed with registration and application to:

    National-Louis University
    1000 Capitol Drive
    Wheeling, Illinois 60090-7201

Please send both the CPS form
and the National-Louis form together.
The CPS form will be forwarded
to the Chicago Public School office for approval.

Enrollment is limited to 25 students on a first come, first served basis.
The forms, with payment, should be returned as soon as possible.

Tuition

• Total tuition is $1155.00.

• Student portion of tuition is $455.00 to be paid by student, due with registration form.

• Check or major credit cards are accepted (see above).

• A $700 stipend is limited to Chicago Public School teachers only upon CPS approval.

• Students order their textbooks from MBS Books:

  • Course number (e.g., MHE501, SCE485, SCE510)

  • Location of class (see Course Schedules on the program information webpages)

  • Instructor name
