	Official transcripts from all community colleges and universities attended

Applying Online

If you APPLY ONLINE, you will be asked to select an Application Type. If you are interested in the BSMIS online program, select Application Type “OL2-On-Line Undergraduate Programs.”

If you are interested in the BSMIS face-to-face (classroom) program, select Application Type “2-Undergraduate Transfer.” Please print the signature page at the end of the application form.

Send the signature page, $40 non-refundable application fee, and transcripts to: National-Louis University Office of Admissions At one of the following addresses:
